Clinical Pharmacist Consultation and Medication Management Program

The Medication Management Program involves pharmaceutical consultation and home visits provided by clinical pharmacists throughout the cities of Burnaby, New Westminster, Tri-Cities (Coquitlam, Port Coquitlam, Port Moody, Anmore, Belcarra), Surrey, North Delta, and White Rock – South Surrey.
Personalized clinical consultation and home visits are conducted by experienced clinical pharmacists. During these visits, patients receive a comprehensive assessment of their medications. The clinical pharmacist will work in collaboration with the patient’s prescriber to optimize medication choices, simplify dosing regimens, and discontinue unnecessary medications. Customized solutions for medication administration such as blister packs or automated dispensing machines may be introduced as needed.

Eligibility: Older adults age 65+ living in the designated service communities residing at home or in assisted living, particularly those who are taking 6+ medications, at risk for medication-related adverse effects, or who have been recently discharged from the hospital. Patients with conditions such as kidney failure, liver failure, dementia, confusion, or recurrent hospitalizations are also prime candidates for this program. Clients residing in nursing homes (Residential Care) are excluded from this service.
